SAVE ALL SHIPPING MATERIAL FOR RETURN!! 

1.   Please use a damp cloth to remove all visible dirt prior to return.  Do not wet hinges or metal clasps.

2.   TO PREVENT DAMAGE properly re-attach headrest and face cushion to the Velcro inside of folded table.

3. IMPORTANT:  REMOVE & DISCARD “SHIP TO” LABEL PRIOR TO ATTACHING RETURN LABEL.      

4.   Return label is attached to shipping box in pouch labeled “Use for return”.  Or, in pouch behind "Ship to" label.  Please fold paper for "Return" label to show through pouch.  If label is lost, discarded, or misplaced additional fee will apply for new return label.

5.   deanna Tee^™ will call and arrange for pick up on “Return Date” submitted on rental agreement.

6.    On “Return date” equipment must be packed and ready when FedEx arrives.  We can make request for pick up time but this is not guaranteed.  Please email or call us if you have specific time requests (i.e. pick up after 4 pm).
